Our Services for Senior Living Communities

Strengthening care from the inside out

We work alongside your team to elevate care delivery, reduce preventable escalations, and provide consistent clinical support that enhances your operations — not disrupts them. Our embedded model is designed to ease the load on Health & Wellness teams while aligning with your culture, systems, and standards. Together, we help meet the growing expectations of residents, families, and regulators — without overextending your staff.

We support communities through:

Onsite clinical collaboration and assessments.

Improved communication pathways with care teams and families.

Reduced risk through proactive oversight.

Tailored workflows that respect your existing systems.

Consistent touchpoints to flag, monitor, and address emerging concerns.

Real-World Impact: Infection Control Without Disruption

On a quiet Thursday morning, one of our partner communities reached out — a few residents had started experiencing sudden nausea, vomiting, and diarrhea. Within hours, the Health & Wellness Director called Savoy Life. Knowing how quickly illness can spread in a communal setting, we immediately mobilized. That same afternoon, our clinical team was onsite conducting targeted assessments and testing 14 symptomatic residents right in their own rooms, helping prevent further exposure that could have occurred during transport to urgent care or the emergency department.

We didn’t stop at just testing. Our team worked closely with the staff to provide anticipatory guidance — a clear list of red-flag symptoms, infection control protocols, and what to watch for during their regular rounds. Within 24 hours, we received confirmation that the illness was norovirus. Thanks to the swift action, the building’s leadership was able to implement focused infection prevention measures before the virus could escalate further.

The outcome? Only one resident needed to be transferred for a higher level of care. Everyone else was supported in place with hydration plans, symptomatic management, and close clinical monitoring. Family members were updated promptly, reassured that their loved ones were being cared for without the distress of hospitalization. The community successfully navigated the outbreak with minimal disruption — and the Health & Wellness director was not managing it alone.

Our Services for Patients & Families

An Extra Layer of Care, Right Where You Live

Savoy Life adds an additional level of personalized care and clinical support — built around the resident, not the system. We partner with your senior living community and your loved one’s existing primary care provider to deliver care that is accessible, proactive, and aligned with what matters most.

We start from day one by getting to know each resident: reviewing chronic conditions, medications, health goals, and any ongoing concerns. From frequent infections to falls or confusion, we work to reduce the risks your loved one faces every day. And when something comes up, we move quickly — either coordinating with their existing doctors or stepping in with one of our own virtual providers, depending on the need.

We Provide:

Personalized, ongoing assessments and chronic condition reviews.

Medication review and care planning in collaboration with your providers.

Quick response to issues with access to our virtual care team.

Family updates and support navigating decisions

A reliable clinical presence that acts as the glue between doctors, staff, and families.

Think of us as an extra layer of care — compassionate, responsive, and fully in sync with your loved one’s community.

Real-World Impact: Restoring Dignity, Rebuilding Life

When Savoy Life first met Mr. Jones, he was quietly suffering. A chronic skin condition had taken over much of his daily life — open lesions made wearing clothing unbearable, so he lay in his room in undergarments on absorbent pads. His door stayed closed. He hadn’t joined others for meals or activities in weeks. With no clear plan for treatment, his quality of life had diminished, and he had nearly disappeared from the social fabric of the community.

Savoy immediately stepped in. Our team consulted with his dermatologist to update his treatment regimen and started coordinating consistent clinical follow-up. During our full-body assessment, we also discovered pressure sores — a result of him spending nearly all hours of the day in bed. When we reached out to home health services, they initially declined the referral due to the chronic nature of the skin condition. But our nurses documented and submitted a detailed wound assessment, highlighting the newly identified pressure injuries. This allowed home health to reassess — and ultimately accept him for skilled wound care services.

Within just two weeks, his wounds began to heal. His pain eased. He started putting on clothes again — and, little by little, venturing out of his room. By the third week, he was attending meals in the dining room. Soon after, he joined a community activity. Today, Mr. Jones is a vibrant presence in the community, chatting with neighbors and engaging in activities. His transformation wasn’t just physical — it was personal. With the right team, the right attention, and the right plan, his dignity and joy were restored.

Our Services for Outside Providers & Hospitals

Bridging Care to Improve Outcomes

We extend the reach of primary care, specialty providers, EMS, and hospital systems by embedding clinical insight and oversight directly into senior living. Our goal is to direct patients to the most appropriate level of care — whether that be ambulatory, specialty, or emergent — and to do so with a full understanding of the patient’s baseline and needs.

We Partner with Providers By:

Identifying and sharing timely clinical observations

Supporting transitions of care and plan implementation

Reducing administrative burden for routine updates

Enhancing adherence to care plans through onsite follow-up

Acting as a trusted extension of your clinical lens within senior living.

Real-World Impact: Right Care, Right Place

After just two months of Savoy Life supporting residents in a regional hub, leadership at a local hospital noticed something remarkable: ER visits from one of the assisted living buildings had noticeably decreased. Even more telling, the emergency clinicians noted that the patients who were sent to the ER arrived with the right level of acuity — clearly appropriate for emergency care — allowing the hospital teams to respond efficiently and without unnecessary workups.

Behind the scenes, Savoy had been embedded in the community, working proactively with residents, families, and staff. Minor issues were triaged and either resolved in place or routed to the appropriate ambulatory or specialty provider. As a result, EMS was no longer the default response. Instead, patients were connected to the right resource, at the right time, with better outcomes and fewer disruptions.

We recognize that in many senior living settings, EMS is often the only available lever. Savoy changes that. By integrating thoughtful, responsive care pathways and being present onsite, we reduce strain on emergency services and create a safer, more coordinated environment for everyone — from bedside to hospital bay.
